1. The total Unity Savings amount for the claim will show as a single line item on your Explanation of Payment. Applies to VSP Signature Plan® and VSP Choice Plan® claims. Qualifying digital lenses include Unity Via Elite, Unity Via Plus, Unity Via Mobile, Unity Via Wrap, Unity Via, Unity Relieve, Unity Via OfficePro, Ethos™ Plus, Ethos, SVx, SVxtra, and SVxtreme. Federal/State Healthcare Program Exclusions: Any item or service for which payment may be made, in whole or in part, directly or indirectly, by any federal or state healthcare program shall not qualify and isn’t eligible for inclusion and/or participation in Unity Savings.
2. Valid through December 31, 2020. Each rewards point has an approximate $1 value toward redemption of items in our VSPOne Rewards Program online shopping catalog. Points have no cash value and must be redeemed by March 31, 2021. VSP Optics reserves the right to change, modify, or end the program at any time.
